Why does deep respiration accentuate the pain caused by appendicitis?

Diaphragmatic excursion (your diaphragm has to move toward the pelvis), causes the pleural surfaces in your abdomen to move relative to one another. In the absence of inflammation/infection, they are slippery and you would not notice anything. When inflammation is present, OUCH!
